This includes and assessment of home safety, medical needs and concerns, cognitive abilities, financial needs and overall fiscal situation, psycho-social needs. This is a very thorough assessment of all aspects of ones life. Please see the three sample assessment forms provided on the website.
Rate: $60/hour - capped at $500
A care plan is a comprehensive document that identifies and instructs the needs and activities to be maintained or initiated for daily living activities. Please see the sample care plan provided on the website.
Fee: $750
Rate: $40/hour
Clients will be billed $10 for every fifteen minutes when you call us, not when we call you.
Rate: $60/hour
This is our standard fee for services provided and includes all of our "services provided."
